Throughout 2012, S+H will go beyond the annual Top 10 by presenting violations 11-30. The data, supplied by OSHA, represents violations cited in FY 2011 (Oct. 1, 2010 – Sept. 30, 2011).
Specific Excavation Requirements
This standard covers regulations regarding surface encumbrances, underground installations, access and egress, exposure to vehicular traffic and falling loads, hazardous atmospheres, protection from water accumulation, stability of adjacent structures, and employee protection from loose rock and soil, as well as inspection requirements and walkway provisions.

Recording and Reporting Occupational Injuries and Illness – Annual Summary
This standard covers procedures for completing the OSHA 300 log at the end of the year and reviewing the log for completeness and accuracy, as well as completing the annual summary (Form 300-A) and certifying accuracy. The 300-A must be posted in the workplace from Feb. 1 until April 30 for the previous year’s injuries/illnesses.
