$69,300 to an Ohio manufacturer for guarding, training and reporting violations stemming from an investigation after an employee’s pinky finger was amputated

$59,290 to a Massachusetts contractor for steel framework and erection violations related to a university building collapse in Rhode Island that injured six workers

$41,200 to a Wisconsin laundry facility for lockout/tagout violations that contributed to the death of an employee who was caught by a conveyor system that propelled into a dryer

$33,800 to a circus operator for allegedly failing to properly erect a tent, resulting in its collapse at a New Hampshire show that killed two audience members and injured several other people, including two circus employees

$19,600 to an Illinois tree service employer for logging operation violations related to the death of a worker who was struck by a falling tree

The Office of Information and Regulatory Affairs – part of the White House’s Office of Management and Budget – reviews proposed regulations. The process is required for most rules before they can move forward, and typically takes 90 days.
