$221,100 to a New Hampshire muffler repair shop for willful and repeat violations stemming from alleged hazards that include electrical shock, defective ladders and unsecured gas cylinders

$170,170 to a Ohio foundry and machining shop for allegations it failed to train and protect workers from noise exposure, respiratory protection and machine guarding

$84,000 to a contractor at a Niagara Falls, NY, worksite for a variety of alleged excavation hazards, including lack of protection to prevent cave-ins
