Alcohol and drug test statement

This “Previous Pre-Employment Employee Alcohol & Drug Test Statement” form can be used to comply with 49 CFR 40.25(j), which requires employers to ask workers applying for a Department of Transportation safety-sensitive position whether they’ve tested positive or refused a DOT pre-employment test and weren’t hired within the past two years.
