$199,800 to a Houston pipe product manufacturer for guarding and lockout/tagout violations in connection with a worker’s struck-by injury

$112,068 to a Connecticut automotive metal forging company for alleged electrical, fall and combustion hazards

$105,490 to a Connecticut polystyrene foam core board manufacturer for Process Safety Management Standard violations

$99,000 to a Louisiana petrochemical plant in connection with an explosion that killed two workers
