3M issues voluntary recall of fall protection sleeve

Red Wing, MN – 3M has issued a voluntary recall of its original DBI-SALA Lad-Saf Sleeve.

In a notice issued Aug. 30, 3M said it recently reviewed the performance of the sleeve in the field – including some instances involving a serious injury or death while the product was in use. The review concluded that although no safety regulator has found the sleeve’s design defective, misuse of the product could lead to serious injury or death.

Misuse can include impeding the product’s braking mechanism through entanglement with cords, clothing or other objects; or grabbing the sleeve before or during a fall. Misuse also can occur when the sleeve is attached upside down.

Impacted part numbers are: 6100016, 6116500, 6116501, 6116502, 6116503, 6116504, 6116505, 6116506, 6116507, 6116509, 6116512, 6116535, 6116540, 6116541, 6116542, 6116500C, 6116500SM, 6116507/A and 6116540b.

3M is urging users and owners of the original Lad-Saf sleeve to stop using the product and quarantine it, and to call 3M Customer Service at (800) 328-6146, ext. 2012, or email [email protected] for information about replacing the product at no cost.

A “completely redesigned next generation” sleeve is replacing the recalled product, 3M stated in the notice.

“We apologize for any inconvenience that this situation may cause you, but we are confident that you will be very pleased with the latest generation X2 and X3 Lad-Saf sleeves to keep your workers safe at height,” the company added.
